All Payments are expected one day prior to your scheduled appointment.
When choosing to use the self-pay option my rate is $150.00 per session
I am concerned about your receiving the treatment that you deserve. Therefore, if you are needing a possible reduced fee, please do not hesitate to contact me.
Insurance:
I am always happy to provide needed documentation for you to submit to your insurance company to obtain partial reimbursement for my services.
If utilizing insurance I accept: Anthem Blue Cross Blue Shield, CIGNA, Traditional Medicare with an insurance supplement, Mercy, Optum/United Healthcare.

Please note the cancellation policy. If you do not show up for your scheduled therapy appointment, and you have not notified me at least 24 hours in advance, you will be required to pay a $75.00 late cancellation fee. All payments are expected prior to your scheduled appointment.
